Case management services are services that meet the ongoing assessment, facilitation, care coordination and advocacy for options and services to meet an individual's needs through communication and available resources, to promote quality and effective outcomes.

Your assigned therapist and/or a specified Case Manager can offer case management services. They will work with you to connect you to any needed services in the community. A Case Manager will offer support between counseling sessions and can assist you with any questions on the “homework” assignments given by the therapist. Case Managers have the flexibility to meet you in the community to provide support in accessing services.
