Contact us today or visit our local office and complete the intake paperwork. We offer same day appointments for an assessment with a licensed mental health professional. Assessments are offered to individuals ages 5 and up. 

The assessment may take up to 2 hours to complete. We conduct a thorough psycho-social assessment to determine need for services and a possible diagnosis. You may be asked to complete several tools or questionnaires to assist the therapist in identifying specific symptoms and a diagnosis. Examples of the tools used are the Generalized Anxiety Disorder- 7 (GAD-7), Post Traumatic Stress Disorder Scale (PTSD-SS), Patient Health Questionnaire-9 (PHQ-9), and other tools as indicated.
